In order for our physician to provide you with the best care, it is important that we gather information about your medical history. All medical records pertinent to your current urologic problem including: labs, diagnostic imaging reports, and office notes should be brought with the patient to the appointment.

We request all new patients to complete our patient registration forms, which can be easily downloaded from our website. Please complete the forms and bring the completed packet with you to your appointment. If you do not fill out the forms in advance, you are encouraged to arrive thirty minutes before the time of your appointment in order to allow ample time to register and fill out paperwork.

Once you establish care with us, we will update your information periodically to make sure our records are current. Our nursing staff will update your medication list and medical conditions with you at each visit before you see your physician. Finally, to comply with federal rules designed to protect your identity, we ask patients to bring their health insurance card (if insured) and a photo ID to each visit.
